) to schedule an appointment to meet with Dr. Lam and his staff. In order to expedite the process, you may elect to download the Encounter Form, fill it out, and bring it with you for your appointment. Also, please download the HIPAA form, read it carefully, and sign at the bottom. This form is to ensure that your patient information remains private. Please bring both of these forms into the office with you to your visit.

Please remember that you will be charged a $100 consultation fee for the time spent with Dr. Lam. This fee may be applied to any other service or procedure that you choose to undertake.
Upon your arrival to the office, you may meet with Dr. Lam's patient care coordinator, Constanze. She may ask some preliminary questions to help make your consultation time with Dr. Lam more effective for you. Dr. Lam will then meet with you and review with you in detail your aesthetic concerns, undertake a pertinent physical evaluation, and answer any questions that you may have. As part of the consultation process, he will try to understand what you are truly interested in. If he believes that you will benefit from some other related procedures, he will discuss with you what he recommends.
With the aging face, there are so many treatment methods that are available. Dr. Lam will help you discover which method(s) would be the most beneficial for you based on your cosmetic interests, budget, desire for surgical or non-surgical treatments, specific anatomy, and projected recovery time.
At the end of the session, you will meet with Constanze, our patient care coordinator, or Marcy, our patient care liaison, to review with you the costs for the procedures that you discussed with Dr. Lam.
Of note, Dr. Lam or his staff will never call to harass you about your decision to book a procedure. You may receive a personal note from Dr. Lam and his staff, but you will not be called unless you contact the office first. We believe in your privacy and support your personal choice in a surgeon.
PREPARING FOR YOUR CONSULTATION
Preparation for your consultation in advance of coming may make your time with Dr. Lam and his staff more rewarding. Viewing the pertinent virtual consultations before coming for your consultation may help you refine some of your thinking, answer some basic questions or stimulate relevant questions after viewing them. Dr. Lam's individual learning modules may also be informative as part of your preparation before your consultation and even serve as reinforcement of knowledge gained during your consultation when you return home.
Dr. Lam has also filmed a series of basic videos to overview his consultation philosophy that will help frame your thinking before you come to the office in order to make sure your time with him is well spent and also provide perhaps a more lucid understanding of his approach toward facial plastic surgery and patient care:

ALTERNATIVE PHONE CONSULTATION FOR OUR OUT-OF-TOWN PATIENTS
Dr. Lam much prefers to speak with you in person for a more detailed evaluation but can do so over the phone if need be. If you cannot fly in for a face-to-face consultation with Dr. Lam, he will require an email from you (sent to

) that shows representative and current photographs of you from the frontal view, oblique (turning 45 degrees) both to the left and right, and at least one profile view.
For nose surgery, please add a view of the bottom of the nose, i.e., with your head tilted back. Try to find a light-colored and clutter-free background for your photographs. Also do not stand next to a window where shadows will obscure your features.
Dr. Lam's patient care coordinator will then schedule a phone consultation with Dr. Lam during which time you can discuss with him your aesthetic interests and concerns as he reviews your photographs. In order to secure this time, you must pay the $100 consultation fee with your credit card. Dr. Lam's patient-care coordinator will then prepare a price quote for you and help to coordinate your care in preparation for a procedure when you are ready.
Before your phone consultation time, please download all of the relevant patient forms including the appropriate patient encounter form and HIPAA privacy form, fill them out, and fax them back to Dr. Lam's office. Our fax number is 972-312-8838. In summary, for your phone consultation with Dr. Lam, we require four things: 1) a $100 consultation fee, 2) standardized, high-quality photographs of yourself, 3) the patient encounter form and HIPAA forms completed and faxed back, and 4) a scheduled time to talk with Dr. Lam in his appointment book.
Although photographs and a phone conversation can help you refine your aesthetic needs and interests, there is no substitute for a personalized in-person consultation in which Dr. Lam can perform a more careful physical evaluation and truly get to know you better.
