Botox is the brand name for botulinum toxin type A (BTXA). Like familiar brands, Kleenex and Coke, most individuals use the term Botox to describe all brands of BTXA, as Dr. Lam does, even though there are now 5 FDA-approved brands for use in the United States. Dr. Lam still favors Botox as his preferred brand for treating facial wrinkles, as he has found it to provide smooth, long-lasting results.

Many individuals confuse Botox treatments with fillers. Botox is meant to lessen dynamic and static wrinkles of the upper face that lessen over time with repeated treatments, allowing patients to space treatments farther apart, whereas fillers are meant to augment volume to the face like for folds and lips. That being said, Dr. Lam does use fillers in a specialized way to complement Botox treatments to address advanced lines that Botox cannot improve, which will be further discussed on this page.

Treatment Overview

1. There are two aspects of Botox that one should keep in mind: a short-term outcome and a long-term outcome. The short-term effect of Botox only lasts about 3 to 4 months when an individual looks exceptionally better. However, every treatment of Botox contributes to a long-term reversal of wrinkles and prevention of further wrinkles. Typically, Dr. Lam recommends a year of treatment for moderate to significant wrinkles and then progressively less often as a preventative measure. 2. Wrinkles will either become progressively better over time with Botox (requiring less and less Botox), or if one does not do Botox the wrinkles will get worse and worse over time. Dr. Lam likes the analogy to exercise and diet: the more someone exercises and follows a healthy diet, the healthier that person becomes; the opposite is also true. One either moves toward greater health or toward less health. Similarly, the younger one starts with Botox the fewer wrinkles one will ever develop over time. 3. The more Botox one does the better and better one’s skin becomes and the healthier the collagen becomes so that even with less and less Botox over time the skin can continue to improve. 4. Botox can also be used for so many other interesting therapies beyond wrinkles like facial sweating, gummy smile, sagging necks, downturned lips, teeth grinding/a wide face, and migraines. Dr. Lam performs Botox for these advanced treatments all the time and is an expert at using Botox for these many types of interventions.

5. Crow’s-feet lines should be treated before they extend down the face, where Botox may have limited effect. Dr. Lam always cautions patients depending on the severity of their current wrinkles not to let the crow’s feet become too deep or the treatment will not be as effective. Many individuals with moderate crow’s feet also believe that the sides of the eyes may not be worth it because the result lasts a bit shorter in duration and may not entirely efface the wrinkles but Dr. Lam believes it is still critical to continue treatments to avoid progression of these wrinkles.

Dr. Lam divides crow’s feet wrinkle severity into 3 grades. Grade 3 wrinkles are hard to completely treat with Botox. When a patient has these Grade 3 wrinkles, however, he believes that using dynamic fillers in combination can greatly improve the results and the longevity of his wrinkle treatments. Is Botox Right For You?

Treatment Process

The treatment for Botox is simple and relatively painless using Dr. Lam’s unique hand vibration technique. After an initial consultation with Dr. Lam including photographs and consent forms, he will then clean and prepare you for the treatment. The injection process only takes about a minute or less to perform and with Dr. Lam’s technique you should feel almost no pain even without the use of numbing cream. You will then be free to leave shortly thereafter.

Botox Recovery Timeline: How Quickly Will You See Results?

There is typically very little to no recovery time following a Botox treatment for facial wrinkles. There will be a bumpy appearance to the skin from the fluid injection that usually lasts about 20 to 30 minutes but may be slightly longer. You may have minor bruising that may last a few days but this is usually minor in scope. There are some restrictions that you need to know. Be careful when reapplying makeup not to disturb the injected Botox for the first few hours. You are not allowed to workout that same day, wear a hat/cap, or lie down and fall asleep. The reason for these limitations in activity is to avoid compression or pressure on the glabellar area (area between the eyes) to avoid inadvertent migration of Botox.

Risks and Side Effects

Botox is generally considered a safe treatment when administered appropriately, and most side effects are temporary. The most common side effect is unwanted bruising, which usually is minor and resolves quickly typically in a few days to a week. Another side effect is that the brows may become heavier feeling or appearing than desired, so Dr. Lam uses a lighter forehead dose for your first treatment, asking you to return in two weeks for a free touch up when he is getting to know your anatomy and how you respond to his treatment. You may also get a headache for a day or two from the Botox, which can happen after the first treatment but is not that common.

There may be other reasons you do not like the result, and Dr. Lam will listen to that and make adjustments on his second round. In rare cases you may suffer an eyelid ptosis (eyelid drop) that can be improved but not eliminated with special eyelid drops and that usually resolves in a month or two. Dr. Lam reports that he has not seen this outcome in his practice in many years and has only had a couple of patients experience this unwanted side effect during more than 20 years in practice. In terms of toxicity or generalized paralysis from Dr. Lam’s Botox, he reports that he has not encountered this complication in his cosmetic practice, where doses are typically low. Another popular question is whether someone could be allergic to Botox, and Dr. Lam reports that he has not encountered this problem in his practice.

How painful is Dr. Lam’s Botox treatment?

For Dr. Lam’s Botox treatment for wrinkles, you will be surprised at how little pain you will experience. Dr. Lam has developed a technique using a triple vibration (his hands and two supporting devices) that makes the experience almost painless, even for people who are afraid of needles. Patients have told him his experience is less painful than even when they had numbing cream, which Dr. Lam does not need. With that being said, his treatment of MesoTox or MesoBotox, used to manage texture and pores of the skin, can have some discomfort during the treatment, and that is why Dr. Lam uses 30 to 45 minutes of numbing cream on the skin.

Does Dr. Lam perform his own Botox or does he delegate this procedure to a nurse?

He performs his own Botox injections even after more than 20 years in practice. He believes that badly done Botox can impact one’s appearance like badly done fillers. He likes to control all of the important surgical (facelift, rhinoplasty, hair transplant) and non-surgical (Botox, fillers) treatments for his patients to ensure that the final result is optimal, natural, and worth it both for short- and long-term results.

What makes Dr. Lam’s Botox treatment better than a medspa or a nurse injector?

Dr. Lam has an excellent aesthetic eye combined with his medical knowledge to help achieve optimal results. For example, if he sees that you have a bad habit of lifting your forehead, he may ease you into a treatment with Botox in the forehead or recommend an upper blepharoplasty, which may be an easier solution to fixing this habit when indicated. If he sees ptosis (weakness of the eyelid muscle), he may also be particularly careful in the forehead, as it may worsen your natural ptosis. Further, Dr. Lam’s treatments really help heal the skin much more rapidly than when you get the procedure performed elsewhere. Many patients tell him that the results they see after one treatment have surpassed what years of treatment elsewhere have tried to achieve. Further, his results appear much smoother and more even than those performed elsewhere according to his patients. He believes he can get a patient to become less reliant on Botox more quickly than when badly done Botox is performed elsewhere. Finally, Dr. Lam follows all safety and sterility practices that are of the highest levels of care at his Joint Commission accredited facility.

How often should I do Botox?

See the Key points section of this page to understand this more in depth. In summary, it is ideal to do Botox every 3 months for the first few sessions to allow time for the skin to repair its own damage. When the static wrinkles, (that is the worn-in grooves apparent in your skin even when you don’t frown or smile or lift your forehead) have almost completely resolved, then you can come in to the office less often for Botox. For men, the goal may be a bit different (see below for the FAQ about that). After the skin has sufficiently healed, you will see that the longevity of Botox is quite a bit longer and you may not need to come in as often over time.

Are there any benefits to Botox other than just for wrinkles in the areas treated for wrinkles?

The key benefit of Botox obviously is wrinkle reduction both short-term and long-term. However, as mentioned, there are many advantages to BTX in its power to heal the skin for texture, pores, tone, etc., in areas treated for wrinkles. That is why Dr. Lam oftentimes combines MesoTox to balance the lower facial skin with the upper treated facial skin. Although some physicians use upper face Botox to treat migraines, Dr. Lam has a far more advanced method to do so, which he discusses on his migraine page.

Is Botox just temporary? Is it worth the money?

Botox is temporary in the sense that after a few months you will be able to move your face again and create wrinkles. However, there are several major advantages that Botox provides long-term for those who want short-term wrinkle reduction. First, Botox literally heals the skin and allows the damaged wrinkled skin to repair itself with the quality of the injection and enough sessions performed. Botox magically heals not only the wrinkles but also the texture, tone, discoloration, and quality of skin in the areas of treatment. That is why people who have religiously undergone Botox treatments but who have significant lower facial sun damage should have MesoTox to make sure there is a balanced skin result in their upper and lower faces, something unique that Dr. Lam has pioneered.

What if I have received Botox long enough that I no longer have wrinkles? Is there still value in continuing treatment?

Even if your skin is flawless, you may still have two benefits with Botox treatments. The first benefit is that every treatment of Botox you do further heals the skin and prevents aging in the treated areas, further lengthening your results. The second benefit is that you should look even more relaxed and improved in your appearance, which can be done before major events like weddings, reunions, etc., even if you do not have many wrinkles left after repeated treatments with Dr. Lam.

I have heard that the more Botox you receive, the more you need. Is that true?

This is categorically false, and the opposite is true. If you perform Botox enough times or start earlier in your life, you will progressively need less Botox. In fact, people who have been consistent with Botox in the past may need very little Botox in the future to maintain their results because their skin has dramatically healed.

What does Dr. Lam think about Botox for men?

Dr. Lam loves Botox for men but has some qualifications. The heavy muscles and tough skin can make Botox slightly less effective in wrinkle elimination than in women. However, men may not seek complete wrinkle elimination but just improvement in wrinkles, which is easier to achieve. Dr. Lam’s philosophy is that some men look better with some mild aging but look worse with too much aging. How far back in time to regress a man depends on what that man wants to achieve. When you achieve the desired level of wrinkle reduction, Dr. Lam can back off. Dr. Lam also offers his aesthetic opinion. If he feels that you are too perfect in your skin, he may counsel you to spread the dosing schedule out. There are some exceptions like men who are in a competitive workforce with a lot of younger men; men who are with much younger partners; and in the gay world a more youthful countenance may be more desirable. What does Dr. Lam think about Botox for actors or people that want a lot of expression?

Botox by its very nature will reduce some motion of the face, so there will be some restriction of activity in facial movement. Otherwise, there will not be a benefit. For commercial actors, this may not be as critical as for dramatic actors. Whatever the case, it is worth discussing with Dr. Lam. If you are pursuing a role where you must appear younger, then he will oftentimes have a plan for you months in advance to help heal the skin, then back off before your audition and/or performance. Will I look frozen?

You should not look “frozen” but you will not be able to frown significantly or raise your brows. The key is not to have unusual movement where there are stray wrinkles that do not look natural. See the next question below. However, Dr. Lam will work with you on how limited you want your movement to be. One thing to note is that his results are very smooth and so there should be a uniform reduction of movement throughout. That is usually considered a major benefit to his technique but in rare cases the reduction may be more than you want. Dr. Lam will do his best to accommodate your requests but will always superimpose his aesthetic opinion and counsel to help you avoid looking unnatural.

How does Dr. Lam avoid the “Jack Nicholson” or “Spock” peaks?

He hates this look where there are stray wrinkles near the edges of the outer brows due to an injector’s fear of injecting Botox in this area. At times he will see this in his own patients, especially after the first treatment where he is more conservative in his forehead injection and where he requests that you come back in two weeks to verify that the dosing is optimal for you. He always encourages you if you ever see this result from him to please come in for a free and quick touch-up treatment.

Isn’t Botox a poison? I’ve heard of a black box warning with Botox? Should I be worried?

No, Botox is incredibly safe with only a few exceptions. The first exception is the rare physician using “fake Botox”, which is incredibly dangerous and corrupt. Of course, Dr. Lam only uses FDA-cleared product. The second exception is in very small children who have compromised lung capacity and who are receiving around 10 times the cosmetic doses on average such as children with cerebral palsy getting large doses of treatment for spastic muscles. This is in fact where the “black box warning” originates from. Dr. Lam has never seen any major adverse side effects that are longstanding in his patients. I am a teenager. Is it too young for me to do Botox?

In many cases, being a teenager is a young age to start Botox with a few exceptions. First, in individuals with significant sun damage, there can be enough skin damage to already justify Botox treatments at that early an age, especially in fair-skinned individuals. Fortunately, the younger you are, the faster your skin will heal and the faster your wrinkles will go away for two major reasons: first, the younger you are the better is the regenerative capacity of your skin; and second, there is usually less damage in the skin when you are younger. It is almost never too young to start Botox, and the earlier you start the less you need it. For example, you may be only doing it once a year or once every other year to stave off aging. With significant sun damage, you may undergo it 2 to 3 times in a year, then back off to once a year or once every other year.

Can Dr. Lam perform Botox at the same time as other treatments?

Dr. Lam can perform Botox at the same time as fillers or other office-based treatments, but the Botox must always go at the very end of an office-based treatment session for a few reasons. First, the fluid in Botox makes it very hard if not impossible to see where the fillers should properly go. Second, Botox treatments should not be physically disturbed or moved after a treatment or there will be a risk of an adverse outcome like a brow or eyelid drop. I have wrinkles that go all the way down my face when I smile, can Dr. Lam fix those with Botox?

Conventional Botox will not address advanced wrinkles and lines that go down the face. Dr. Lam uses a dynamic filler to help also improve these lines by filling in these lines. He will then use Botox afterwards to help further improve these lines and to maintain the result longer. When I do Botox for crow’s feet, the result does not last that long. Any suggestions?

I only want to treat my forehead lines. Is this a good idea?

Usually, the answer is no except in a very young individual. The reason is that the frontalis is the only elevator of the forehead, and when it is blocked without treating the crow’s feet or the glabellar frown lines, the risk is that the other muscles, which are depressors, will pull the forehead down, especially in someone who usually raises his or her eyebrows because the brows are already heavy. I hear a Botox crunching noise when I am being injected with Botox. Is that normal?
